BACKGROUND CHECK CONSENT FORM

DISCLOSURE

NWI Coyotes may request and use consumer reports and/or investigative consumer reports in connection with your application for volunteer employment. These reports may include information about your character, general reputation, personal characteristics, and mode of living.

AUTHORIZATION

I hereby authorize NWI Coyotes Youth Football & Cheerleading to obtain consumer reports and/or investigative consumer reports regarding me from a consumer reporting agency. This authorization is valid for current and future reports, and I understand that this consent will remain valid throughout my employment with NWI Coyotes.

I acknowledge that I have received a copy of the Summary of Rights under the Fair Credit Reporting Act.

NOTICE REGARDING CRIMINAL HISTORY INFORMATION

In accordance with Illinois law, NWI Coyotes will not inquire into your criminal history until after you have been determined to be qualified for the position and a conditional offer of employment has been made.

If an adverse decision is made based on your criminal history or other information obtained in the background check, you will be provided with a copy of the report, an explanation of the decision, and information on how to dispute the accuracy of the report.
